On May 15, 2003 at the Richmond Convention Center, VIPC&S re-convened 450 Virginia leaders from diverse disciplines who share a commitment to advancing patient safety and quality. These individuals explored how effective and open dialogue between all parties can advance safety improvement and accountability; how technology and best practices reduce medication error rates, improves service delivery, reduces cost, improves quality and increases accountability.  On May 16-17, the Executive Fellowship in Patient Safety held its on-campus sessions (see presentations and posters).  Carolyn M. Clancy, M.D. , Director of the Agency for Healthcare Research and Quality served as this year's graduation speaker.
